Prenatal Care Coordination (PNCC) is a Medicaid and BadgerCare Plus benefit that helps pregnant women get the support and services they need to have a healthy baby. PNCC services include:
- Help getting the health care they need
- Personal support
- Information on good eating habits and health practices
- Help finding needed services in their community
The purpose of the Medicaid Prenatal Care Coordination program is to provide access to medical, social, educational, and other services to pregnant women who are considered high risk for adverse pregnancy outcomes. The components of this benefit are outreach, assessment, care plan development, ongoing care coordination and monitoring, and health education and nutrition counseling. Some key outcome indicators include: Tobacco Exposure, Alcohol Use, Breastfeeding, Safe Infant Sleep Practices, Perinatal Depression, Family Planning, and Involved Father.A Case Manager is responsible for providing (PNCC) Prenatal Child Care Coordination Services and ongoing (CCC) Child Care Coordination Services to Clients as mandated by the Wisconsin Department of Health and Human Services. The Case Manager will work with mothers and the recipient's family to promote positive parenting, assist in improving low birth outcomes among women who are deemed at high risk for poor birth outcomes, and prevent child abuse and neglect
This position _ PREFERS_ a Bachelor's degree in Social Work, Psychology, Nursing, Occupational Therapy, Criminal Justice, Human Services - OR - a Bachelor's degree in an unrelated field with experience in Human Services, preferably Case Management
Responsibilities and Duties
- Recruit, enroll and maintain a caseload of approximate 15-20 families currently residing in Milwaukee county.
- Establish a trusting relationship with high-risk families by delivering service that is culturally competent, strength-based, family centered, and family directed.
- Conduct family assessments with each parent to determine strengths and weaknesses.
- Conduct weekly home visits with all enrolled families to improve healthy pregnancy outcomes, parenting skills and to help them provide learning skills that enhance their child's growth and development.
- Coordinate Prenatal Care Coordination Services with prenatal families.
- Complete family assessments either prenatally or at the baby"s birth, and develop and implementIndividual Family Support Plans based on family needs and preferences.
- Provide assistance to families to increase competency in the areas of positive parent-child interaction, home environment, safety, nutrition, child growth and development, and child healthcare.
- Document all contacts and information, and ensure each family file is current
- Act as an advocate for parents and children in the community.
- Working from home and meeting clients in the community.
Qualifications and Skills
- Must have a vehicle, and maintain a valid Wisconsin Driver License, and adequate insurance coverage
- Must have GOOD WRITING SKILLS and an ability to meet program deadlines.
- Proficient in use of computer systems and internet, to include Microsoft Office, Word, Access, and Excel
- Knowledge of community resources and capacity to maintain strong working relationships with collaborating agencies.
- Knowledge of child abuse and neglect prevention, maternal child health and child development.
- Experience with client services which embraces family support concept and demonstrates attitude that is accepting, non-judgmental and culturally competent.
- Strong organizational, communication and computer skills.
- Respect for families of diverse socio-economic status, race, ethnicity and ability to engage in dialogue regarding personal issues.
