Under the sponsorship of Hamilton-Madison House and in partnership with The Community Agency for Senior Citizens (CASC) assist Staten Islanders by bringing together community resources to provide case management services. Case Management Program provides case management services, per NYC DFTA Standards for older adults, age 60 years and older, who are residents of Staten Island. Conduct outreach to Asian population on Staten Island to ensure awareness about the program. We do this by working with the older adults, respecting their right to make decisions for themselves, honoring their life experiences and heritage, and recognizing the importance of friends, relatives and community to create supportive environments for people as they age.

Responsibilities:

  • Oversee a team of 2 case managers servicing older adults with functional limitations throughout Staten Island.
  • Provide support, training, and quality assurance of intakes, initial in-home assessments and re-assessments of clients, including closing cases as appropriate and within the DFTA Standards time lines.
  • Assess at risk seniors and provide crisis intervention as needed. Collaborate with Program Director in determining safety plans.
  • Support staff in authorizing EISEP homecare and housekeeping, and home delivered meals as appropriate and in keeping with eligibility requirements.
  • Responsible for Supervisory Review of case records for clients assigned on the NYC DFTA STARS client database system.
  • Provide leadership to team and program including individual supervision of case managers and facilitating team meetings, group supervisions and trainings.
  • Collaborate with community service providers including advocacy and coordination of care.
  • Conduct outreach in the Asian community as needed.
  • Maintain a familiarity of NYC DFTA Standards, including updates to standards and procedures.
  • Participate in all required DFTA Trainings and required hours of ongoing training as set by NYC DFTA Standards.

Minimum Qualifications:

  • Master’s Degree in Social Work or related field
  • 3-5 years of supervisory experience in health/human services, working with older adults preferred
  • Proficiency in Microsoft Word, Excel and Outlook required
  • Experience with STARS database (preferred)
  • Bilingual: English-Chinese required
  • Must have a car or use public transportation for travel for work


Hours: Monday to Friday, 35 hours/week (Some evening and weekend hours may be required in emergency situations)
