Position Summary:
Cooperative Health strives to achieve outstanding health outcomes. Health outcomes are demonstrated through data metrics. Each data metric is successfully realized when a clinical team member delivers a prescribed service and the service is successfully recorded in Athena (electronic medical record (EMR)), the data metric data points are aggregated into a report. When a prescribed service is not delivered, the service is not successfully recorded in Athena, or the data metric is not aggregated into a report, then a gap in care is created. Cooperative Health and insurance partners strive to minimize gaps in care.
Under the supervision of the Director of Clinical Quality Improvement, the Clinical Quality Improvement Outreach Coordinator will serve as a liaison between office team members who deliver health care services, Cooperative Health’s Health Information Technology Department (Health Analytic Technologist / Managed Care Coordinator and Population Health Specialist), and Insurance Care Organizations.
Principal Accountabilities/Responsibilities:
- Works with Cooperative Health’s Population Health Specialist to maintain a full understanding of all clinical quality measures and successful recording of the measures in Athena
- Works with Cooperative Health’s Population Health Specialist, Health Analytic Technologist, and the Director of Clinical Quality Improvement to maintain a full understanding of all clinical quality HEDIS measures and successful recording of the measures in Athena
- Works to achieve the highest level insurance company based incentive program payments
- Will work with the CMO, applicable Clinical Care Committee team, and Insurance companies to develop unique strategies to reduce unnecessary emergency room utilization.
- Assist clinical team members to develop best practice workflow to ensure maximum patient medication adherence.
- In collaboration with the Director of Clinical Quality Improvement and representative from UnitedHealthCare will regularly detail all applicable clinicians and medical offices about this UnitedHealthCare initiative.
- In collaboration with the Director of Clinical Quality Improvement and the Finance Department will maintain a running detailed report on all incentive payments received and from which companies the payments are received
Education & Experience:
- Education

- BA – Health Care Administration or health related field
- RN or LPN
- Experience

- Technology skills include familiarity with health care management, basic spreadsheet, and database software / systems
- Knowledge of continuous quality improvement principles and the ability to understand and use analytical reasoning to interpret and plan action on patient care data trending
- Excellent interpersonal communication and problem solving skills
- Excellent written and oral communication skills with an emphasis on presentation skills
- Knowledge of statistics, data collection, analysis and data presentation
