Provides comprehensive nursing assessments, develops intervention strategies, and provides nursing services to individuals, families, groups, and communities; provides case management for high-risk clients.
An employee in this classification may perform any of the following duties. However, these examples do not include all the specific tasks which an employee may be expected to perform.
May perform any of the duties of the Public Health Nurse 1, but distinguished by a primary focus on the duties listed below.
- Responsible for a defined program, or service area in carrying out prevention, intervention and services for identified health concerns of individuals, families, communities, and or schools.
- Solicits, initiates and promotes activities that maintain the health of individuals and families, through teaching, counseling, and appropriate preventive and wellness measures.
- Performs comprehensive holistic assessments, including assessing the needs of individuals, families, and communities.
- Determines nursing diagnosis through use of screening and assessment.
- Interprets health-related information to families, enabling them to integrate this information into their daily lives. Identifies community resources and services available to assist families to meet individual needs.
- Develops and implements comprehensive plans of care, to include such intervention strategies as:
- Education focused on health promotion and disease prevention.
- Counseling, coaching and supporting the individual or family to reach mutually identified goals.
- Case management activities which assist the family is accessing and utilizing available services.
- Family advocacy which includes a family centered, holistic approach to the management of complex care.
- Evaluation and redevelopment of the nursing plan to determine the effectiveness of the intervention strategies.
PROGRAMS:
The Public Health Nurse II will support one or more of the following programs:
- Communicable disease: investigate reportable communicable diseases through telephone case interviews, contact tracing; act as a consultant for outbreaks in congregate facilities, education, prevention strategies
- Clinical services
- reproductive health/STI: utilize standing orders and protocols to provide STI screening, treatment and education for clients; utilize standing orders and protocols to provide birth control options for clients, client education and referrals
- Immunizations: utilize standing orders to provide immunizations to clients, client education and referrals
- Home visiting programs:
- Babies First!: case management and care coordination for pregnant women and infants/children ages 0-5 with social risk factors
- CaCoon: case management and care coordination for children with special healthcare needs age’s birth through age II0.
- Universal Home Visiting Program: RN assessment of infant/mother dyad during first month postpartum, client education, care coordination
Performs related duties as required.
Graduation from an accredited school of nursing with an associate’s degree in nursing, BSN Preferred. Experience in public health, hospital or a clinic setting is preferred, but not required.
Possession of a license to practice as a Registered Professional Nurse in the State of Oregon at the time of appointment. Must possess and maintain a valid Oregon driver’s license and automobile insurance, and have access to and use of a personal automobile as a condition of employment.

Health Insurance (medical, dental, Rx and vision): We offer Kaiser Permanente and PacificSource. Coverage varies according to bargaining unit. Employee share of the premium will vary according to bargaining unit and full-time or part-time status.
Paid Leave:
- Vacation: New full-time employees generally earn 12 vacation days per year; this increases with years of service. Part-time employees earn a pro-rated amount.
- Holidays: A generous number of paid holidays annually. This will vary according to bargaining unit.
- Personal Leave Days: These are floating holidays. The number of Personal Leave Days varies according to bargaining unit.
- Sick Leave: 8 hours per month (pro-rated for part time) with no maximum accrual.
- Other Leaves: Jury duty, bereavement, military.
Retirement: We participate in the Public Employees Retirement System (PERS). Polk County picks up the employee's share of the retirement contribution (6% of wages.) See the PERS website for more information: www.pers.state.or.us
