An RNAC is accountable for coordinating and overseeing the full collaborative, interdisciplinary assessment and care planning process in a skilled nursing facility. The RNAC supports person-directed care and facilitates this through the assessment and care planning process.

REQUIREMENTS: Must have one-year experience in long term care with a working knowledge of MDS, care planning, and PPS rules.
EDUCATION: Registered Nurse licensed in the state of Pennsylvania.
