What your day might look like:
- Ensures coordination and integration of residential, rehabilitation and clinic treatments (behavioral and primary care) for residents
- Provides supportive and educational support services in accordance with OMH regulations
- Provides required documentation in a timely manner in accordance with Federal/State regulations and BHSN policies/procedures
- Supports residents with daily needs including, but not limited to transportation, linkage to other services, care coordination and discharge planning
- Observes the self-administration of medication according to established protocols
- Develops service plans following the eleven restorative services as required by Office of Mental Health regulations and documents progress
- Assists/provides support to residents with various aspects of daily living skills
- Additional duties as assigned within scope of job description
Your skills and qualifications:
- High school diploma or equivalent required
- Experience in the human services field preferred
- Strong interpersonal, communication, time management and organizational skills
- Proficient in Microsoft Office software
- Ability to climb stairs; ability to lift up to 25 lbs
- Valid NYS driver’s license, acceptable to insurance carrier, strongly preferred required local travel
- CPR certification required within 60 days of employment
- Ability to perform duties such as changing a light bulb, washing dishes, vacuuming, dusting, raking, light shoveling, sanding/salting outdoor steps/walkways, etc.
