Job Title: Behavioral Health Provider

Department: Healthcare for the Homeless

Reports To: Mental Health Program Manager

FLSA Status: Non-Exempt

Schedule: Mon-Friday 8am-5pm; Full or Part-time availability

Wage Range: $29.98-$42.26 hourly; depending on number of years of transferrable experience and internal equity

SUMMARY: The behavioral health provider (BHP) offers integrated behavioral health in the Healthcare for the Homeless primary care clinic. Care is team based and the BHP consults with medical providers and other clinic staff to provide the “warm handoff” of patients at the point of care. BHP is highly available and is required to understand the primary care culture. The majority of the BHP work is consulting with medical providers and patients to provide solution focused treatment in the management of mental health and substance use issues, chronic disease management, and lifestyle change needs.

ESSENTIAL KNOWLEDGE, SKILLS AND RESPONSIBILITIES include the following. Other duties may be assigned.

  • Provides individual, couples, family, and group counseling and assistance to program participants and their families in accordance with prescribed program policies, rules, and regulations.
  • Coordinates educational materials to program participants.
  • Provides on-going emotional support and acts as an advocate for clients with other community support services and organizations.
  • Provides assessment to determine appropriate counseling and intervention methods.
  • Performs a variety of community outreach services, including conducting informative workshops, and providing information, advice, and counsel to other community agencies, referral sources, and the general public as requested.
  • Maintains client records and documentation in accordance with program requirements.
  • Recognizes and educates staff on managing patient care is a team effort that involves clinical and nonclinical staff (e.g. physicians, nurse practitioners, physician assistants, nurses, medical assistants, educators, front desk staff, schedulers, etc) interacting with patients to achieve stated objectives.

EDUCATION AND EXPERIENCE

  • Master’s Degree in behavioral health field (Psychology, Counseling, Social Work) or a closely related field;
  • Considerable clinical experience specific to the functional area of assignment preferred.

CERTIFICATES, LICENSES, REGISTRATIONS:

  • Valid State of Montana Driver’s License
  • Current Montana professional license in good standing (LCPC or LCSW)/ Dually Licensed with LAC preferred.
