PathPoint is seeking a full-time Service Specialist to join our Behavioral Health Division in Santa Barbara, CA. The position is currently funded through a startup grant from the Department of Health Care Services through December 2023, with the goal of the program becoming self-sustaining. The position is 40 hours per week with flexible schedule; driving is required.

In collaboration with CenCal Health, PathPoint aims to decrease historical barriers certain groups have faced to access physical and mental healthcare. As a Service Specialist, you will be responsible for the quality, effectiveness, and efficiency of two CenCal programs: Enhanced Care Management (ECM) and Community Supports (CS). These two services aim to provide comprehensive care that addresses the clinical and non-clinical needs of the highest-need members that are enrolled in Medi-Cal. The intent is to streamline care provision, reduce access disparities, and improve care quality.
In this role, you will provide case management for an assigned group of clients, while acting as a liaison between client and treatment agencies to meet client and program objectives. You will assist clients in learning household maintenance and educate and encourage clients to utilize community resources, and support systems. As a Service Specialist, you will ensure our services are holistic, client-centered, and trauma-informed. The client's background, such as race, ethnicity, socioeconomic status, past housing history, gender identity, etc., are considered to create a treatment plan that best resonates with them. You will ensure the use of client-centered tools, like motivational interviewing, to address critical mental and physical health issues (including substance use), promote ongoing health maintenance tasks (such as visiting the doctor, exercise, and medication management), and increase housing retention.
What We're Looking For:
- High school diploma or GED required.
- BA or drug alcohol certificate preferred.
- Must have one year experience working in health and human services with individuals with mental illness, substance use disorders, or with people who are without homes.
- Must have a valid California Driver's License and satisfactory DMV record.
- Must meet all legal requirements to bill Medi-Cal
- Must receive a criminal record clearance through the DOJ and pass pre-employment health and TB screenings.
- Culturally sensitive and aware of differences in employees based on cultural backgrounds; aware of one's own culture and how someone's background may lead to biases; has the skills and resources needed to work with employees from all different cultures and the ability to avoid making biased assumptions based on someone's culture.

Compensation: The starting pay and pay range during the grant period is $23.50 per hour. If the position becomes self-sustaining after December, 2024, based on available funding, the pay range is expected to be $23-26.
