Direct Reports: Recruitment & Care Coordinator/Case Manager to include the Leads, & Administrative Specialist(s)

Availability: You must have open availability - Our shift starts as early as 8:00AM and can last throughout the evening. Must have open availability on the weekends. If you have a hard stop time or a second job, this position may not be for you. However, if you are hard-working, can motivate a team, lead and produce results, this position is for you.

Summary: This role is to ensure continuity of care for the consumers as it relates to Recruitment and Case Management. This role will be responsible for executing the growth action plan, may also act as the liaison between the Support Coordination ODP Organization under ODP and the provider to ensure that all individuals receive the necessary supports and services they require and desire while apart of the agency’s program and fill any vacancies as necessary. 6100, 6400 and Day Program Regulations

You must be familiar with Charting the Life Course, In-Home and Community Supports, Community Participation Supports, Companion and Job Supports, Day Program along with their codes and Certified Investigations. You must also have the ability to convert units and hours for budgeting. You will oversee Recruitment, Care Coordinator/Case Manager, and their Leads/Admin Assistant. The Quality Assurance will be responsible for:

Referrals: We have a goal to support 2250 hours per quarter or more

Program Planning & Compliance:

Become proficient in the responsibilities below, so that you can become proficient in Supervising and conducting them

  • Facilitate the implementation of Individual Service Plans (ISP) and participate in a range of administrative duties including staff hiring, terminating, training, scheduling, conduct performance evaluations and oversight.
  • Scheduling Shifts
  • Facilitate community involvement by accompanying individuals on outings in the community or providing transportation to work or other activities
  • Ensure that the agency, including all staff are in compliance with 6100, 6400 and Day Program regulations
  • Reviewing progress notes
  • Scheduling, contacting and developing volunteer opportunities for the individuals
  • Developing schedules for the individuals

Recruitment/Onboarding:

Become proficient in the responsibilities below, so that you can become proficient in Supervising and conducting them

Education/Experience:

Bachelor's Degree (B. A.) from four-year college or university preferred.
