We are a state-of-the-art primary and specialty care facility in the Chelsea area of NYC.

We have flexible work schedules, holidays off, and a collaborative team environment.

Work Schedule: (10:30 am to 6:30 pm) or (8:30 am to 6:30 pm) Monday -Friday.

Looking for LPNs fluent in Spanish who would like a two-day, three-day, or five-day workweek.

Salary $ 60,000 -$70,000 Based On Experience

The LPN provides direct patient care and supervises Medical Assistants in the assigned clinic area including case finding, health teaching, and counseling under the direction of the RN.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

1. Interprets for and communicates with physicians and all health care providers in providing patient care.

2. Supervises Medical Assistants.

3. Functions as a Case Manager by facilitating the delivery of individualized and coordinated care. Provides recommendations to administration in the improvement of patient services and clinic systems.

4. Administers injections, and medications, and performs treatments as per physician orders and UHC policies and procedures.

5. Directs and assists in the opening and closing of the clinic including daily clinic assignments and preparing, rooming, and exiting patients for provider visits as needed.

6. Reports and records information regarding teaching and patients’ response to treatment.

7. Identifies patient goals for consideration by the RN for possible inclusion in the patient care plan.

8. Maintains availability and readiness of all equipment and supplies in assigned areas.

9. Assists in orientation of nursing staff including Medical Assistants and LPNs.

10. Assists the RN in the completion of performance evaluations and competencies of Medical Assistants.

11. Assists in the coordination of patient flow within the clinic.

12. Coordinates response to patient emergencies and initiates immediate action.

13. Supports and reinforces policies regarding patient care, infection control and safety.

14. Participates in Quality Assurance activities.

15. Complies with the Union Health Center’s policies & procedures related to privacy and security of identifiable patient information (PHI), as explained in the HIPAA (Health Insurance Portability & Accountability Act) training.

SUPERVISORY RESPONSIBILITIES

Supervises Medical Assistants.

QUALIFICATIONS

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. The Union Health Center will comply with applicable standards of the Americans with Disabilities Act.

1. New York State Licensed Practical Nurse.

2. Two years of ambulatory experience as an LPN preferred.

3. Strong interpersonal, oral, written communication, and collaborative skills.

4. Computer literate. Knowledge of EMR is a plus.

5. Bilingual English/ Spanish required.
