Maryland Treatment Centers Inc., provides a comprehensive continuum of treatment for persons of all ages with problems involving substance use. We feature specialty services for adolescents, young adults and adults. Clinical programs include assessment, residential/inpatient, detoxification, day treatment, intensive outpatient and outpatient substance abuse, dual diagnosis, mental health, special education and prevention services. Our programs are accredited by the Joint Commission (JCAHO).

Avery Road Treatment Centers is currently seeking a State Care Coordinator. The overall responsibility of the State Care Coordinator is to work closely with the Care Coordination supervisor to insure participants ability to access services needed and to establish recovery in their community.

Responsibilities:

• Obtain a bio-psycho-social assessment, and other documents needed for Care Coordination
• Orientate client to Care Coordination, at orientation appointment, become familiar with client’s recovery plan and access authorized services
• Complete electronic consents and referrals for contacting other service providers
• Create authorizations (including Care Coordination monitoring units) schedule appointment to administer intake MDRN
• Set up all future appointments, and how client can best be contacted. Contact client bi-weekly via telephone or face to face.
• Complete discharge at the time of the recipient’s discharge from the last remaining Recovery Net Service, using the MDRN.
• Performs other duties and responsibilities as assigned. (Responsibilities listed above are a general description of the level and nature of work assigned to this position and is not to be considered as all inclusive.)
