Overview:
The Care Transition Coordinator is responsible for generating hospice referrals through professional relationship with all referral sources, which may include physicians, nursing homes, assisted living facilities, and hospitals. Must also develop relationships within the community through activities and events while maintaining strong organizational skills, clear communication, and an effective understanding of the hospice philosophy.

Territory
: Hattiesburg and surrounding areas (Perry, Marion, Covington, Jeff Davis, etc)
Responsibilities:
  • Establish and promote positive professional relationships with physicians, hospitals, skilled nursing facilities, discharge planners, and case managers within an assigned territory to promote our hospice services.
  • Educate the community, referral sources, patients and families on hospice services through daily, planned activities including in-services, one-on-one presentations and group meetings.
  • Develop, execute and evaluate strategic marketing plans including tactics to meet budgeted admission goals.
  • Within scope of position, provide leadership and personal accountability to ensure compliance to company and regulatory policies, business growth, professional development and integrity.
  • Collaborate with the patient care team to provide assistance in obtaining information from referral sources and handling stressful situation and deadlines by effectively communicating with the healthcare team.
  • Gather and organize account-related information and provide input on key customer opportunities, and proposed contracting.
  • Take initiative to develop and maintain relationships with team members to assure effective communication both internally and externally for our referral sources.
  • Ensure compliance to regulatory requirements and guidelines, company identity and admissions standards.
  • Utilizes marketing/sales collateral to maintain and develop business opportunities.
  • Assist the program as needed with informational visits with patients and families.
  • Plan and coordinate public education and communication efforts to increase community outreach
  • Stays within the budgeted expenses set by management for all in-services and activities.
  • Conduct on going meeting with physicians, discharge planners and other healthcare professional.
  • Meet and/or exceed goals set by sales leadership.
  • Maintain timely submission of timesheet, expense reports and CRM documentation.
  • Read and adhere to all Agency policy and procedures.
  • Participate in in-service education and other personnel training.
  • Ensures compliance with all regulatory agencies governing sales referrals and other sales related activities in accordance with Stark Laws, Anti-Kickback laws and company policies.
  • Attends all sales meetings and training session required by sales leadership.

Environmental/Working Conditions: Performs duties within the assigned territory by making visits to referral partners, including, but not limited to physicians, facilities, e.g., Assisted Living Facilities, In-Patient Rehab, acute care hospitals, participating in community events, etc. May be required to meet with patients and/or their caregivers to provide education on home care services available. Possible exposure to blood and bodily fluids and infectious diseases; the ability to work a flexible schedule; the ability to travel locally; and some exposure to unpleasant weather.


Skills: Ability to establish and maintain effective working relationships with referral partners, all segments of the staff, the lay and professional public, and the Medical Director.


E nvironmental and Working Conditions: Ability to travel day and/or some overnight travel, approximately 90-100% of the time or as needed within the Division Service area. Ability to work a flexible schedule; ability to travel locally; some exposure to unpleasant weather.
Qualifications:
Education: Bachelor’s Degree in marketing or Business preferred and/or experience in a health-related field or equivalent.


License: If functioning in a clinical capacity, must have a current license and be in good standing with the licensing Board.


Experience: Minimum of two years health care or related industry sales experience. Has a working knowledge of hospice care and the different aspects that are involved in supporting both the field staff and leadership of the program. Intimate knowledge of Medicare Hospice Certification and for the incumbents own professional requirements.


Skills: Excellent communication skills and the ability to effectively communicate hospice services to the community and healthcare professionals. Be coachable with a goal to grow personally and professionally. Embrace the hospice philosophy and the principles of death or dying. Ability to work independently and prioritize. Has an ability to deal tactfully with the community. Understands hospice care and the services provided to patient and family/caregiver through an interdisciplinary group.
