• BadgerCare Plus - CCHP is offered in twenty-eight (28) counties in eastern Wisconsin, and covers more than 145,000 adults and children, making it the second largest BadgerCare health plan in the state. With the combined resources of Children's Hospital, Children's Community Services, Children's specialty and primary care clinics, and our community partners, we are committed to improving the medical and social well-being of our members and the community.
• Care4Kids - Care4Kids is Wisconsin's Foster Care Medical Home and is a voluntary Medicaid benefit that provides comprehensive health care coordination services to 3,000 children who were placed in out-of-home care in Southeastern Wisconsin. The program supports the unique health needs of these children and aims to improve health care outcomes by strengthening the quality, access, and timeliness of care utilizing a "medical home" model approach for each child who enrolls in the program. Care4Kids provides children with access to primary care physicians who are trained in the needs of children in out-of-home care, and also creates a team of professionals who coordinate care for the child.
• Together with CCHP - Together with CCHP is a health insurance plan for individuals and families in a 14 county service area in Wisconsin. Together with CCHP currently serves 16,000 members and offers access to high-quality health care from our broad network of providers including Children's, Ascension, Froedtert & the Medical College of Wisconsin, Bellin Health and others. Together with CCHP is available on and off the exchange providing members with comprehensive coverage and programs to meet their health care needs.

General Summary:

Assists Chorus Community Health Plan (CCHP) members with complex and chronic conditions utilizing problem solving techniques, referral to appropriate community resources, and/or referral to internal case management. The Outreach Coordinator will work closely with the clinical team of nurses and social workers to assist with decision making.

Minimum Knowledge, Skills and Experience:

• Bachelors Degree and 2 years of experience in health promotion, health advocacy, health education or community outreach.
• Knowledge of the Medicaid population and benefits preferred.
• Requires a valid driver's license, acceptable driving record, personal auto liability insurance coverage and the ability to be insured by the CHHS auto insurance carrier.
