Job Description:

  • Bachelor’s degree in counseling, psychology, social work, or related mental health discipline
  • Prefer some experience working with individuals with mental illness
  • Ability to work independently and part of a team is required
  • Belief in the concept of mental health recovery
  • Must have own reliable transportation, liability insurance, and TN driver’s license
Summary of Job:
Provide case management services for children in foster care. Provide health home services to behavioral health consumers and families by coordinating the treatment of behavioral health conditions (mental health & substance use) and physical health conditions; by providing comprehensive care management; care coordination; transitional care; health promotion to improve overall wellness and manage chronic health conditions; and monitoring health indicators
Essential Duties and Responsibilities:
  • Ensures participants understand how to successfully navigate the health care system by providing education and support to them and their family/caregivers
  • Ensures participant`s behavioral and primary care is coordinated by collaborating and actively communicating with service providers and along with the participant develops strategies to remove barriers
  • Provides health promotion and assists participants and their families/caretakers in developing self-management skills to improve their health
  • Ensures that participants have access to social supports such as transportation, housing, benefit information, and learns the skills they need to access them independently
  • Utilizes effective behavioral management techniques, crisis intervention, de-escalation, active listening, motivational interviewing, etc.; demonstrating knowledge and skill to provide quality services.
  • Designs and implements appropriate consumer specific service plans, crisis plans, etc.
  • Displays knowledge and understanding of ethical standards by evidence of practice with clients and community members.
  • Responds promptly to client’s needs to coordination of services
  • Completes documentation as required
  • Achieves targets related to participant contact
  • Compliance with annual KPI evaluation
  • Regular and reliable attendance is required
  • Other duties as assigned
