ESSENTIAL FUNCTIONS

• Completes required documentation for client to successfully access all eligible VA benefits • Tracks applications process, coordinates any additional information with VA representative • Monitors benefits with team meetings, staffing, interaction with other team members as well

• Completes all required case management forms including documentation and maintaining updated required audit information and maintaining HMIS records confidentially • Coordinates client's ability to access other community services and completing the referrals for their usage

• Submits TFA’s monthly for rent/utility financial assistance to be approved by supervisor, maintain monthly budget records with clients, maintain monthly TFA tracker, uploading copy of the check, TFA and supporting documents to client file

• Completes referrals for services in the community and coordinates access to those services needed

• Measures the household's progress toward self-sufficiency through the HUD Self Sufficiency Matrix which will be administered at program entrance, every three months and at program exit

• Facilitates housing placement process for each client in the community including housing counseling, landlord relationships, affordable and safe housing selection as well as a monthly budget

• Conduct inspections of potential housing units and provide documentation • Attending all required meetings and trainings as directed by supervisor

• Maintain personal safety and adhere to all safety policies and procedures • Any and all other duties assigned by supervisor

• Bachelor's Degree in social work or related field is required or nonprofit management or related field, or (10) ten years' experience in program leadership position in a nonprofit setting; Master's degree preferred

• Extensive organized case management experience through written verification and verified documentation

• Intensive case management and experience with families experiencing homelessness with children is preferred and clients with substance abuse and/or mental illness

• Working knowledge of VA benefits system, including eligibility requirements, applications, appeals, VA file documentation and Bi-Lingual is preferred

OTHER REQUIREMENTS:

• Clean driving record

• Strong interpersonal skills, effective judgment in interviewing clients and coordinating services with communities' agencies. Processing all information in a confidential manner

• Good writing skills for program planning, reporting and correspondence. Ability to use independent judgment, perform research, investigations and inspections and recommend solutions

• Ability to think and react calmly in emergency situations
