Position Title: Lead Medical Case Manager

• Flexible 1st shift hours, M-F full time
• This Lead position provides case management services to our highest acuity clients living with HIV. The primary function of this role is to address barriers to care ensuring clients are able to prioritize their HIV medical care.
• Located at the Mcclees Clinic on the Hackley hospital campus

Position Summary:

MSW Medical Case Management is the provision of a range of client-centered activities focused on improving health outcomes in support of the HIV care continuum. The MSW MCM Lead is a professional who collaborates with physicians, health care team members and patients to coordinate care for specific patient populations across the continuum. Provides various social work services to meet needs of patients including leading groups and mental health crisis interventions in addition to routine medical case management activities: biopsychosocial assessment and reassessment, care plan, insurance navigation and linkage to other resources necessary for patient engagement in care. Provides mentorship, guidance and support to other case managers. Social work practices and services are consistent with applicable accreditation and licensing standards, and reflect the National Association of Social Worker Code of Ethics.

What the Lead Medical Case Manager will need:

• Masters of Social Work. Prior employment experience in a related area preferred. Demonstrate ability and skill in the management of patient care planning and continuous quality improvement in services delivered.
• MSW, LLMSW, or LMSW
• Experience in case management and experience/interest in facilitating groups.
• Comfortable driving clients to apt. as needed; limited only on occasion
