EAC Network, a not-for-profit social service agency that empowers, assists, and cares for over 54,000 people in need through 100 programs across Long Island and NYC, seeks a Case Manager - Medication Assisted Treatment (MAT) to work full time for its Queens Felony Drug Court program

The Case Manager Medication Assisted Treatment (MAT) position is a non-exempt position paying $21 per hour at 35 hours per week. The hours of operation are Monday-Friday 9:00AM-5:00PM and the office is located in Queens, NY. The role reports to the Program Director.

PRIMARY PURPOSE OF JOB:

The Case Manager/MAT utilizes discretion and judgment to provide screening, treatment planning, case management, and court reporting.


PRINCIPAL DUTIES AND RESPONSIBILITIES:

  • Act as a liaison to the Courts. Provide screening services and related services for cases within the judicial system for clients involved with alcohol or drug abuse and/or who present evidence of mental illness in order to ascertain appropriate treatment as an alternative to incarceration.
  • Follows and implements a comprehensive screening to effectuate referrals based on an individual assessment to mental health, drug abuse, or alcoholism facility and accompanies and/or arranges transportation for clients to the appropriate facility when necessary.
  • Assist clients in applying for and receiving public and private benefits.
  • Maintain case documentation available for review by supervision at any time, i.e. client records, including progress notes and all other relevant information to assist clients and to comply with program requirements.
  • Maintain a caseload of clients and monitor their participation in the treatment program through case management appointments, reviewing progress reports, and conferencing cases with treatment personnel.
  • Provide special focus on clients needing medication-assisted treatment (MAT) and coordinate with treatment providers to determine the appropriate treatment plan.
  • Prepare and present written and oral status reports to the criminal justice system in a timely manner regarding client's progress.
  • Review clinical assessments of clients for the purpose of reporting and determining the appropriateness of client for groups and programs; evaluate clients for chemical dependency issues; assess changes in the client's condition/treatment needs.
  • Investigate and arrange supportive services for clients.
  • Make timely visits to treatment facilities.
  • Conduct educational and/or supportive groups.
  • Conduct drug testing on clients of the Program as needed.

MINIMUM KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED:

  • Bachelors degree in criminal justice, social work, counseling, forensic psychology, psychology, sociology, criminology or a CASAC or four years of relevant experience required.
  • Good communication skills, both oral and written. Ability to work cooperatively with treatment and criminal justice systems.
  • Ability to use computer applications or other automated systems such as spreadsheets, word processing, calendar, and e-mail and/or database software in performing work assignments.
