Job description
CRi, the region's premier clinical services provider for people with mental health needs and developmental disabilities, is seeking Case Managers for our Tenant Support Services Team!
The Case Manager is responsible for implementing and guiding the delivery of all supportive services provided to individuals who have leases within the Wesley Housing designated PSH units. The Case Manager conducts assessments, develops service plans and provides crisis supports as needed. The Case Manager meets with individuals, facilitates groups and opportunities for fellowship and learning while also mentoring and supporting Tenant Support Specialists in the execution of skills training and supports delivered to individuals. The Case Manager works closely and collaboratively with Wesley Housing staff in the complexes and hubs to ensure a collaborative working relationship with the goal of individuals maintaining housing and abiding by all expectations therein.
Principal Duties and Responsibilities:
- Completes assessments.
- Develops service plans.
- Develops programming for individuals geared toward skills training, education and social determinants of health.
- Demonstrate sound clinical skills including those associated with advanced communication skills.
- Develop essential therapeutic and collaborative relationships that foster engagement in services, housing stability, independence and supports the change dynamic and recovery process.
- Teach skills necessary to obtain and maintain housing while supporting independence and overall recovery.
- Interact with government, community agencies and individuals to develop consumer network referral base.
- Assess individual's strengths, needs and support systems to facilitate the development of individual service plans.
- Assist/instruct consumers in implementing their individual service plans.
- Perform crisis intervention.
- Mentor and support Tenant Support Specialists in the implementation of service plans.
- Work independently, performing position duties under general supervision.
- Communicate effectively, verbally and in writing (e.g., documentation).
- Maintain all clinical and programmatically required documentation in compliance with all relevant regulations, contract standards, and/or CRi policies.
- Establish and maintain ongoing supportive relationships with individuals.
- Performs other duties as assigned by supervisor.
Minimum Qualifications Required:
- Bachelor's degree in a human services field; preferably social work or counseling field.
- Must have a minimum of one (1) year of experience with assessments, service plan development and service delivery.
- Minimum of one (1) year experience providing services to individuals with SMI and I/DD diagnosis.
- Must have a valid driver's license.
- Must be knowledgeable of the following topics:
- Treatment modalities and intervention techniques for population identified, to include crisis intervention or support; independent living skills training, supportive counseling, group work, case management and service coordination.
- Clinical assessments and their use in service planning.
- Consumers' rights.
- Local community resources and service delivery systems.
- Effective oral, written and interpersonal communication principles and techniques.
- Social determinant of health.
- Record keeping with EHRs, all documentation.
- The service planning process and major components of a service plan.
Physical Demands:
- Should be in good physical condition for required lifting and physical intervention for hazardous behavior and assistance with care.
