CNA - In Center

Community PACE at Home is looking for compassionate and skilled Certified Nursing Assistants to serve our elderly participants. Join our growing team of home care professionals. These positions are available full-time. We need to provide care for our participants within our facility. The Program of All-Inclusive Care for the Elderly (PACE®) model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in the community whenever possible. We are looking for caring, compassionate, individuals to join our team of health care providers at Community PACE at Home in Newaygo. We are fun and fast-paced.

These positions are full-time, Monday through Friday starting at 8am – 4pm. There are no mandates, no weekends, and a relaxed setting.

Job Summary and Specifications

• Communicates effectively with participants, families, staff, and the staff of other agencies.
• Participates in interdisciplinary team meetings.
• Rides along with participants on bus routes.
• Works effectively in a team setting.
• Provides basic ADL nursing care (showers, eating, etc.) to assigned participants.
• Documents treatments/procedures, observations, nursing care, and participant activities on appropriate forms and in the medical record as appropriate.
• Participates in the process of teaching and learning with others.
• Variable hours.
• Provides personal care in the center
• Abides by all established Community PACE at Home policies, rules, and regulations.

Certified Nursing Assistant Requirements

• CNA License
• Previous home health care experience, preferred
• Ability to communicate effectively, both written and verbally
• Strong ability to use critical thinking and education when performing job tasks
• Ability to consistently connect with patients one-on-one
• Responsible, compassionate, and caring demeanor

Job Type: 1 Full-time position available - In Center

Pay: $14.00 - $16.50 per hour (Compensation is based on experience)
