GENERAL PURPOSE
The Clinical Response Coordinator (CRC) functions as the leader of the Clinical Response team which works in partnership with Department of Developmental Services (DDS) to provide support services for people who e require high levels of behavioral healthcare in medical, family and community placements. The Clinical Response Coordinator will also supervise the team as they work to support stabilization in community placements to prevent the need for a more restrictive setting. The Clinical Response Coordinator also works with Community Program administrators to oversee the review and response to all referrals received from DDS for Clinical Response Team Referrals. In addition, the CRC may be consulted regarding clinical needs for other individuals receiving services in the Oak Hill/Gilead system.
DUTIES AND RESPONSIBILITIES
  • The Clinical Response Coordinator is responsible for managing the day-to-day operations of the Clinical Response Team (CRT).
  • Facilitates the process of gathering and dissemination relevant records and other information to the Clinical Response Team members in a timely manner. Facilitates informational meetings related to this process between CRT members and other providers.
  • Works in cooperation with clinical staff to ensure appropriate assessment of clinical issues and design of interventions.
  • Facilitates development of comprehensive plan for treatment, placement, training, and all delivery of services in order to ensure successful transitions through the CRT process, from initial assessment to final placement.
  • Facilitates dissemination of related documents and training of necessary staff. The CRC coordinates training of Direct Care Staff, clinical staff from other agencies, family members, and others as necessary.
  • Facilitates CRT communication, meetings and ensures outcome goals, including discharge, are met through team process.
  • Participates in weekly rounds and twice monthly Regional Director meetings with DDS to review the current status and progress of individual receiving CRT service (with other CRT members as appropriate)
  • Works with DDS and members of the CRT to identify appropriate supportive settings for individuals when appropriate. This may include available openings in DDS, Oak Hill, Gilead Community Services, other providers, or alternative community sites. The emphasis will be on finding the most appropriate placement available, considering factors such as family location, availability of resources in each location, safety of the participant and others, etc.
  • Ensures that all necessary data are collected and compiled to best direct treatment measures, demonstrate effectiveness of interventions, and demonstrate delivery of services.
  • Works to develop new CRT program parameters and procedures as necessary.
  • Familiarity with Oak Hill, DDS, Department of Child and Family Services (DCF), Department of Public Health (DPH), and Department of Mental Health and Addiction Services (DMHAS) policies, procedures and requirements which relate to the Clinical Response Team, to ensure their implementation as appropriate.
  • Ensures that all DDS, other relevant agencies, and Oak Hill policies and procedures are followed.
  • Hiring and training Clinical Response Clinicians as appropriate.
  • Supervision of Clinical Response Clinicians.
  • Data collection and management.
  • Documentation support and supervision.
QUALIFICATION REQUIREMENTS
Minimum Requirements: Master’s degree in Social Work, Applied Behavior Analysis, Psychology, Education, Human Services, or related field with 2 years or more years of experience in delivering services to people diagnosed with Intellectual Disabilities, Mental Health needs, and behavioral challenges.
Preferred Candidates will have a master’s or Doctoral degree in Social Work, Applied Behavioral Analysis, Psychology, Counseling, or Education, and be licensed in Behavior Analysis, Counseling, Psychology, or Social Work, or working toward such qualification.
