Community Health Worker - Medical Appointments
Job description
Work in unison with community and clinical partners to provide comprehensive and coordinated Community Health Worker interventions. Grant funded position.
- Assistance with scheduling and attending in-person and virtual medical and specialty appointments with consumers.
- Help consumers set personal goals, and attend appointments successfully.
- Work closely with medical provider to help ensure that consumers have comprehensive and coordinated care.
- Work cooperatively with other clinical and social service agency personnel assigned to the same patient.
- Assist consumers with completing applications, registration or other forms as necessary.
Education/Experience Requirements:
- Associates degree in human services or similar.
- Driver’s License and reliable vehicle.
- Community Health Worker Core Competency Training within two years of hire or at discretional timeline as approved by Supervisor.
- A minimum of two years of community, clinical experience and/or volunteer experience.
Job Type: Full-time
Pay: From $20.00 per hour
