Specific Responsibilities:
This is a Senior Community Health Nursing Supervisor position assigned to the Northeast Health Center Refugee Program Clinical Services and Jupiter Auxiliary Health Center. Responsible for the day-to-day activities and supervision of the clinical staff providing direct service to Refugee, Latent Tuberculosis (LTBI), Immunization Clinics. Monitor and manage LTB I/Refugee/Immunization clinic flow to minimize wait and discomfort for patients served at this facility. Work is performed under the supervision of the Community Health Nursing Consultant. This position will be managing staff responsible for sensitive information and financial responsibility. This position requires independent judgment, initiative, applying general knowledge of business practices and skill in applying instructions to accomplish various job functions. Responsible for the supervision of staff including appraisals, planning and directing their work, approving leave and having the authority for hiring and taking disciplinary action or effectively recommending such actions.
This position is designated as a sensitive position and is required to maintain confidential information in accordance with the Florida Department of Health (FDOH) Palm Beach County Health Department (CHO) Information Security Policy, Protocols and Procedures. Confidential Data Set: Clinical Data, Immunization Data, HIVe & TB, Epidemiology (7) HIV/AIDS Case Reporting and Surveillance, School Medical Record, WIC, Employee Health Record, Surveillance Tools & Reporting System, FL SHOTS, Intranet In-Stock Inventory System (SIS), Electronic Health Record (EHR) Pharmacy Forms System (PFS), HMS Report Portal, Q-Flow Health Management System (HMS) for the purpose of reviewing patient medical records, printing of schedules, report portal to view/print for Refugee/LTBI/Immunization Programs, reviewing patient medical visits and or services, Fla Shots for entering and updating immunization records or printing immunization records, HMS for Refugee Assessment Module to review and print progress notes and reports, HMS TB Module to review and print progress notes and reports. This position may view, update and release 'CONFIDENTIAL' patient information and print data from HMS for continuity of care purposes based on established guidelines.
Supervises direct reports including motivating, training, providing guidance, and direction. Develops expectations and meets with staff regularly to discuss performance, complete performance reviews, and discipline action, as needed. Maintains vacation and work schedules to ensure proper coverage and completes timesheets. Ensures all staff complete required training and are offered additional training.
Provides direct care through the provision of required core Refugee Health Assessment. Follow-up services on clients of all ages in accordance with current Refugee Health Program guidelines. Placed on communicable disease screening; orders laboratory tests; performs extended services as clinically indicated. Update services provided in the HMS Electronic Health Record; Document client service and other information on hard copy forms as needed; assures patient materials on health education, information about local community health resources, and guidance on accessing the health care system are provided. Makes appropriate referrals for general and specialty follow-up; Reviews laboratory test results and assure clients with abnormal results are contacted.
Ensure efficient flow of the LTB I/Refugee/Immunization Clinics by assigning fair and appropriate workload to various staff while maintaining excellent patient care, ensuring ample clinic coverage at all times.
Oversees the timely billing of services. Act as the liaison for the Refugee Health Screening Program with the State Refugee Program. Attends the county Refugee Task Force meetings as needed.
Participates in developing, revising nursing procedures and ensures nursing standards are maintained. Provide new staff with clinic and nursing orientation and facilitate student preceptorship. Participate in center management meetings to discuss changes and exercise knowledge within practice areas and disseminate new information to subordinate staff. Ensure case management is initiated and completed by assigned staff. Ensure broken appointments are followed up by appropriate staff.
Maintains on-site State Pharmacy services for LTBI including but not limited to, medication ordering, inventory control and QI audits. Responsible for patient profile forms and hard copy prescriptions, medications, accurately labeled all distributed medication and ensure Provider's signature when applicable. Responsible for an internal data base to re-order the patient specific medications, a perpetual up to date inventory of pharmaceuticals. As a clinic supervisor the incumbent will ensure that all nursing staff that has access to this pharmacy is trained appropriately and any error that occurs is reported immediately to the Center's Administration and to the State Pharmacy.
Coordinates clinic function with multidisciplinary staff providing services for clinic patients. Maintains current knowledge and practice of vaccine management and immunization assessment, administration and follow-up. Directs immunization services, as appropriate, at the Northeast and Jupiter Health Centers. Reviews patient records on an ongoing basis to ensure accuracy of documentation content and completion.
Maintains communication with Refugee, TB, and Immunization Program managers to coordinate clinical services as needed. Assists with bringing delinquent patients into treatment.
Participates in scheduled team meetings for discussion and management of patient care. Participates in planned in-service education sessions and Quality Assurance program and all assigned staff meetings.

Qualifications:
- This occupation requires licensure as a Registered Professional Nurse in accordance with Chapter 464, Florida Statutes and in accordance with Rule 6489, Florida Administrative Code.

Emergency duty required of the incumbent includes working in special needs shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.

- This position may travel to provide services at another satellite Center, work Saturdays and or a weekly late evening clinic on a rotation basis at the discretion of Administration through the Health Access Office. May be required to flex any hours exceeding regular work schedule.
