Job description
The CHC Complex Care Team works collaboratively to improve patient health outcomes through educating, advocating, and providing supportive assistance with and between clinical teams, families and caregivers. Health Center patients are cared for by a regular team of health care providers and support staff. The team assesses patients to determine level of risk for hospitalization, uncontrolled chronic disease, or other co-complicating factors which may affect reaching their desire health care goals. Upon referral, complex care works closely with all members of the team to formulate, implement and communicate care plans toward the improved health. The manager will oversee and lead these efforts as well as expand CHC remote patient monitoring program.
BSN with 5 years+ experience in direct patient care. Care Coordination and/or chronic disease self-management experience strongly preferred
