The Connection Leader manages the day-to-day operations in various Medicaid Waiver-funded locations, ensuring that all support provided meets the expectations of the people supported and all state and federal requirements. To do this, you will provide supervision and support to Direct Support Professionals working in PAC and Respite Services. You will provide case coordination services by entering goals and completing quarterly summaries. The Connection Leader will also be a mentor and support to people with Intellectual and Developmental Disabilities to live the life they choose to live.

Requirements: Preferred Minimum Qualifications

• Individual must be of unquestionable character as determined by personal references and background checks. Applicant must supply sufficient recommendations to support the fact that maturity, integrity, and dependability are hallmarks of his/her personality.
• Preferably one full year of successful experience working with people with Intellectual and/or other types of Developmental Disabilities.
• Must have a high school diploma or equivalent.
• Must have the ability to effectively express one’s self in written reports, including electronic reporting of documentation.
• Past supervisory experience desirable.
• Must have good communication skills.
• Ability to project a professional image while contacting community resources.
• Must have a valid driver's license and acceptable driving record.
• The ability to communicate effectively, both verbally and in writing, with all levels of co-workers to promote an effective teamwork approach to carrying out the responsibilities of the position, programs, and agency's goals.
• Ability to physically restrain, lift and/or assist clients.
• Ability to occasionally climb stairs.
• Must have the availability and willingness to take calls or assist in working as needed outside your normal scheduled hours
