Job description

The designated coordinator will provide necessary supervision and leadership in the home, to ensure that the daily needs of each consumer is met (including physical, emotional, self-care, and daily life). The designated coordinator will work as a team member, taking a turn in the rotation with carrying an after hours cell phone to address the needs of the homes when the office is not open.

QUALIFICATIONS

Degree in a field related to human service and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older and one year of experience in a field related to human services, in a supervisory capacity.

And

• Ability to clear a criminal background study.
• Good clean driving record and maintain current driver’s license.
• Capable of working without direct supervision. Ability to understand and implement Lakes Homes’ policies and procedures.
• Ability to maintain confidentiality and professionalism.
• Ability to assess basic medical conditions and implement basic 1st aid/CPR

RESPONSIBILITIES

• To enhance human worth and potential of consumers served by Lakes Homes and Program Development.
• Maintain a current understanding of the licensing requirements sufficient to ensure compliance throughout the program as identified in statues 245A.04 and 256B and 245D regarding coordination and evaluation of individual service delivery.

Work Location:

• Will be based out of Fergus Falls, MN or Detroit Lakes, MN

Work hours:

• Generally Monday through Friday, but must be flexible to meet the needs of a residential environment. You may be required to work some evenings and weekends.

Job Type: Full-time

Pay: $18.00 - $24.00 per hour
