You’ll lead a team of discharge planning and utilization management professionals – experts in helping to transition our community back to post-acute care and home.
The Director of Inpatient Case Management:
- Is responsible for the overall management of the department's daily operations to achieve quality improvement goals, fiscal productivity, patient satisfaction goals and objectives established by upper management.
- Has overall responsibility for staff performance and the development of departmental goals in accordance with the hospital, case management practice and regulatory requirements.
- Collaborates with all disciplines, departments, payers, system partners, vendors and community agencies.
***Regular commute to the Albuquerque, NM metro area is required for this position.
***Relocation assistance is available.
Qualifications:
- Bachelor Degree in Nursing
- Registered Nurse - NM or compact license
- Certified Case Manager (CCM) or obtained within 1 year
- Minimum 3-5 years of inpatient case management leadership experience and/or acute care nursing leadership experience.
- Prior case management leadership experience (at Manager level or above) is required.
- Develops departmental long-range and short term capital and operating budgets to support hospital and departmental mission.
- Partners with the case management physician advisor (medical director) and fosters an effective relationship with physicians and ancillary team members.
- Partners with ICS leadership, clinical directors, business office and PHP leadership to foster case management at an organizational level.
- Participates in the determination and definition of the case management dashboard, using appropriate data to determine needs, decisions and strategies.
- Develops, implements, and monitors evidenced- based practice and assures quality care is delivered to all patients.
- Establishes, manages and evaluates staffing levels, productivity, assignments, skills, learning needs and development case managers, social workers and health service assistants.
- Oversees and participates in annual and interim performance reviews for all staff in the department.
- Contributes to, mentors and coaches ongoing education and professional development of case management department and acts as a resource for staff.
- Produces and maintains required tracking and trending reports in accordance with the department and regulatory requirements and department quality assurance programs.
- Performs personnel management functions including hiring, coaching, counseling, evaluating and terminating.
- Facilitates effective communication between administration, staff, physicians, other ancillary departments, patients, families, and community resources. Participates in conflict resolution as necessary.
- Maintains current knowledge of information in case management and related fields through seminars, workshops and publications.
- Performs other functions as required.
