DIRECTOR, POST ACUTE SERVICES | WFH/Remote

Preferably Work at Home out of Ohio or Virginia

Travel to OH, SC, and VA quarterly

The Director supports the post-acute service lines and post-acute clinical programs on behalf of the CIN/PHSO. This will include both owned and affiliated post-acute providers, skilled nursing facilities, home health, and hospice organizations. Through this role, BSMH will maintain effective relationships that produce highly reliable care, patient experience, and provider engagement.

Essential Functions

• Lead the development of the criteria for establishing a coordinated care network for post-acute providers to include but not limited to skilled nursing homes and home health agencies. Supports the development and execution of the Post-Acute strategy for the PHSO / CIN and for Mercy Health

• Supports the development and implementation of the PHSO services; content and expectations of the PHSO Value-Based Care Playbook

• Acts as Liaison to support the continuity of care process with the Post-Acute facilities and services, while increasing collaboration with hospital case managers, care transitions coordinators, ambulatory care coordinators and external resources to ensure smooth transitions to post-acute care sites

• Supports improvement activities regarding continuous quality improvement within the Post-Acute Care services line, including statistical and comparative data analysis; provides guidance on developing and implementing key performance indicators and reviews on a regular basis

• Monitors Post-Acute network provider performance on a regular basis and identifies opportunities for remedial action.

• Maintains current knowledge of practices, and regulations, and proactively responds to changes within the Post-Acute Care industry

• Supports the CIN/PHSO’s performance on bundled payment initiatives by aligning post-acute services for enrolled patients.

• Support the PHCO/CIN with work with the at risk contracts including data analysis of performance and opportunities for improvement.

REQUIRED:

• Bachelors Degree in Healthcare Administration, Business or Clinical Practice
• 5+ years of post-acute leadership or business development experience

PREFERRED:

• Masters Degree in Healthcare Administration, Business or Clinical Practice
• Clinical Licensure in Nursing, Respiratory Therapy, Rehab Therapy, Social Work
• Experience with Post-Acute Care specific performance metrics strongly preferred
