Youable Emotional Health has an opening for a Diversion Outreach Worker which can directly impact the lives of children, ages 10-17, and their families who are first time non-violent offenders in order to reduce involvement in the juvenile justice system. In this role one will assist in locating hard to reach youth in the community and encourage their engagement in the Diversion process.

The Diversion process is a holistic approach of assessment looking at risk for further offenses, school disengagement, family disruptions, and mental and chemical health struggles. Once assessment is complete, youth are referred to supportive services within their community to address concerns identified, as well as asked to complete community service and provide restitution, if required. Youth are monitored and supported by the Diversion Case Manager through completion of the program.

Location: Travel between the Richfield office, Brooklyn Center office and community locations, as needed.


Hours:
The position is 40 hours per week and varies between the hours of 9am - 8pm, Monday - Friday.

ESSENTIAL JOB FUNCTIONS

Youth engagement

  • Engage youth and families in the Diversion process through phone calls, face to face meetings, and connecting clients/families to community resources as stated in client's Diversion contract. Be available to clients via phone as needed.
  • Complete outreach home visits to engage youth and families in the Diversion process

Case management services

  • Completes assessments and initial screening with youth and families to determine needs and risks
  • Develop diversion contract with youth and families
  • Develop restitution contracts with youth and victims
  • Provide safety and crisis planning
  • Make referrals to community supports and services
  • Provide transportation for youth and families to Diversion appointments, as needed
  • Monitor school attendance for students who have an identified attendance concern
  • Complete school visits and develop plans to address attendance concerns for identified students

Develop and maintain partnerships with community partners

  • Maintain relationships with community partners and schools
  • Collaborate with partners at the Hennepin County Attorney's Office
  • Attend Diversion meetings at the Hennepin County Juvenile Court as needed

Paperwork and agency responsibilities

  • Completes all documentation and data entry required for the program in a timely manner
  • Attends consultation, staff, and other meetings as directed by the supervisor or agency policy
  • Follow and enforce all agency policies and procedures
  • Perform other duties as assigned

QUALIFICATIONS

Required


  • Associate's degree from an accredited college or university
  • Job/internship experience and training in working with youth and families
  • Experience and training in identifying and assessing a wide range of adolescent issues and needs
  • Knowledgeable about local community resources and how to utilize these resources to meet client needs
  • Must have their own car for work
  • Valid driver's license and insurance
  • Able to work evening hours

Preferred, but not Required

  • Bachelor's degree in one of the behavioral sciences from an accredited college or university
  • Work with youth involved in corrections

Knowledge, Skills and Abilities

  • Proven ability to establish and maintain positive rapport with a diverse variety of clients and professionals in school and community settings
  • Ability to develop and maintain rapport with diverse and challenging youth
  • Demonstrated excellent verbal and written communication skills
  • Demonstrated ability to collaborate and build relationships
  • Proficient computer skills in Word and Excel programs
  • High degree of organization and ability to work independently and take initiative
