Job Role Summary

Directs engagement and linkage activities and case finding initiatives within the Ryan White HIV Services Program. This position provides direction, guidance, training, and assistance for the Engagement Liaison team who are responsible for finding and linking individuals into HIV care and support services. Incumbent plays a team lead role in helping diagnose, treat, prevent, and respond to end the HIV epidemic. This position will work to increase knowledge about HIV disease for clients, new employees, and the community being served. As the team lead, this position will coordinate implementation of EHE programs, policies, and procedures. This position will be funded through HRSA Ending the HIV Epidemic (EHE) Cooperative Agreements, and continued employment is contingent upon receipt of the grant.

Essential Duties

  • With direction guidance from the EHE Grant & Quality Management Manager works to create and conduct daily operations, activities, and initiatives of the Engagement team.
  • Provides direct day to day instruction and guidance to the Engagement Liaisons.
  • Maintains expert knowledge of state-of-the-art client case finding, engagement and linkage techniques.
  • Coordinates with the EHE Data Specialist on case assignments, maintenance of referrals, client engagement tracking, and in-house evaluation of engagement activities.
  • Coordinates with RWSP MAI team via routine huddles to synchronize client engagement, testing, and linkage services.
  • Regularly reports updates to the EHE Grant & Quality Management Manager on the activities, progress, challenges, and needs of the Engagement team.
  • Required to perform all Engagement Liaison responsibilities including client engagement when staffing circumstances necessitate
  • Provides a variety of comprehensive HIV presentations to the identified populations, service providers, communities, professionals, and other health and service entities that are responsible for the care, financial allocation, or advocacy of enrolled or eligible clients.
  • Develops and maintains relationships with agencies that provide HIV Counseling and Testing to provide a link between initial HIV diagnosis and linkage into HIV care and services.
  • Writes, reviews, and edits comprehensive reports that document the overall success, challenges, outcomes, and trends of the Engagement Liaison services.
  • Performs phlebotomy and other specimen collection procedures in the field and in a clinic setting for patients in need of testing
  • Supports the MCPHD vision and mission through department presence in the community.
  • Assists in the coordination and mobilization of community partnerships to identify, strategize, and solve health problems related to Ending the HIV Epidemic (EHE) and the populations served.
  • Establishes and maintains effective lines of communication and collaboration with the community, targeted populations, and other program areas within the MCPHD.
  • Assists in the development of program objectives that target those in the community most at risk and/or most in need of care, training, and skills development.
  • Participates in public health emergency preparedness exercises and response to public health emergencies, as directed.
  • Participates in two H.O.P.E. events per year.


Other Duties and Responsibilities:

  • Serve on appropriate committees and task forces as assigned.
  • Participation in Health Fairs.

Qualifications

  • Bachelor’s Degree in Public Health, Social Work, Political Science, Education or other Social Sciences, and allied health field required.
  • Minimum of two years of professional experience in client engagement and linkage or qualifying experience
  • Knowledge of HIV/AIDS, STI/STD, and/or Public Health related issues
  • Prior supervisory or team management experience desired

Knowledge, Skills & Abilities

Language Skills:

  • Excellent interpersonal, oral, and written communication skills
  • Ability to read and interpret documents such as medical information, laboratory results, public health laws, safety rules, operating and maintenance instructions, and procedure manuals.
  • Bilingual applicants are a desire (i.e., Spanish, but not a requirement of employment)
  • Capacity to collaborate with patients and staff regardless of age, race, gender, sexual orientation, and /or religious affiliations.
  • Basic computer skills including the ability to utilize database, spreadsheet, webinar, power point, and word processing software.
  • Provide effective leadership to staff when situations require a coordinated rapid response.


Reasoning Ability:

  • Ability to analyze data to identify problems and to devise and implement corrective actions.
  • Ability to solve practical problems and deal with a variety of complex situation in short time frames.
  • Strong organizational skills, including attention to detail, multi-tasking, establishing workload priorities, and meeting deadlines.
  • Ability to work independently and in team relationships effectively.

Working Enviornment

  • General office setting in health department facilities, as well as community sites including clients’ homes or other workplaces.
  • Valid Indiana driver’s license
  • Exposure to communicable disease, illness, and irate community members.
  • Some physical effort associated with filing, carrying supplies for health fairs and/or exhibits, travel between main office and meetings, program site(s), and health fairs.
  • Required to drive both in-county and out-of-county for the coordination of program initiatives.
  • May be required to work evenings and weekends.
