Duties and Responsibilities
- Advocate for individual or community health needs with government agencies or health service providers.
- Maintain an individual case-management load of 25 patients, as needed. In some situations this could be reduced or increased to meet organizational needs.
- Attend community meetings or health fairs to understand community needs, build relationships with community members, and provide information on accessing available services.
- Build effective working relationships with individuals and community members & utilize multiple strategies and skills to perform outreach to identify community resources that may serve them in the following general areas:
- Basic (i.e., emergency shelter, housing, transportation, food access, etc.)
- Mental health (i.e., psychiatry, therapy- individual and group, substance use, crisis services, peer support, etc.)
- Wellness (i.e., integrative or holistic care, primary and preventative care, dental care, etc.)
- Support and advocacy (i.e., health care/insurance and other benefits, legal, etc.)
- Quality of life (i.e., recreational resource, spiritual or religious organizations, vocational supports, etc.).
- Develops and maintains an up-to-date database of resources in the Middlesex County and collects data regarding population served.
- Coordinate and facilitate educational workshops and programs on mental health and addiction related topics to educate and support community members on overcoming social difficulties, improve health, prevent addiction, etc.
- Provides general administrative support and coordination for all aspects of client care, inclusive of but not limited to, filing, document preparation, client scheduling and telephone support.
- Facilitate initial phone or in-person screening of the client’s needs, inclusive of but not limited to, mental or medical health, substance use, cultural, spiritual, and physical ability to help identify appropriate resources to meet those needs.
- Maintain updated client records with plans, notes, appropriate forms, or related information in accordance with HIPPA regulations.
- Maintains and implements the Recovery Core Principles with direct implications to staff, clients, family, and community development.
- Identify & proactively engage members of high-risk or otherwise targeted groups, such as minority populations, low-income populations, or pregnant women, via various outreach efforts to address and limit negative outcomes of social-determinants of health.
- Actively engages in facilitation of MHRC’s a family education programs.
- Ensures that the program’s physical environment meets required regulations including a clean, safe, and welcoming atmosphere. Gilead Community Service
- Bachelor’s degree in human services, psychology, allied health services, sociology, social work, or similar field preferred.
