Calling all Health Navigators, come join our Center of Excellence - Refugee team! We are seeking a Health Navigator that speaks and writes Spanish fluently, and is able to use their language skills to engage with clients, provide resources and supports, and bridge with community partners. The ideal candidate works effectively as part of a trauma-informed, culturally response team providing services for youth and adult of all ages.


About the team.

The Health Navigation team works in partnership with the Refugee Clinic to provide culturally responsive, trauma-informed services to New Americans settling in the Aurora Community. The Health Navigation team is comprised of staff who have expertise in serving the diverse Aurora community through the provision of case management, interpretation, and care coordination services.


Salary for this role.
*wage is based on experience and company equity.

  • $18.89 - $28.33


Bilingual Language Differential Pay.
*rate depends on language requirements for the position and candidate's fluency.

  • 10% of base salary up to a maximum of $6,000/year


About the role.

The Spanish speaking Health Navigator will provide case management, interpretation, and care coordination services to immigrant and refugee members of the Aurora Community. They will work closely with therapists to provide high quality, culturally responsive care in Spanish.


Responsibilities:

  • Operate as an integral part of a behavioral health team.
  • Provide language interpretation to clients receiving behavioral health services such as individual counseling, medication follow up, psychosocial skill building and wellness groups.
  • Provide first hand cultural knowledge as a member/leader in local refugee/immigrant community in Colorado. This includes helping to identify, understand and address the needs of refugee/immigrant communities.
  • Ability to build trusting relationships with community members and act as advocate for refugee community and liaison with diverse network of providers.
  • Provide outreach & health education to immigrant/refugee communities and share information about full range of programs, services and how to access them.
  • Facilitate client and family engagement and optimize communication between clients and providers. Assisting with transportation, reminder calls, encouraging regular attendance at appointments, care coordination and case management.
  • Receive ongoing training and team support from supervisors, therapists, peers and medical providers. Complete patient navigation and interpretation training as needed.
  • Help conduct initial screenings/interviews for immigrants/refugees identified as at-risk for behavioral health issues.
  • Help translate health and mental health education material into primary languages as needed.
  • Connect clients and their families to various community resources, referrals, advocacy, service planning. This may involve telephone contact, home visits, and meetings.
  • Provide cultural education/training for providers.
  • Assist with other tasks and projects as needed, including grants.
  • Staff member will monitor and coordinate care for assigned clients in partnership with the other members of the clients care team.

Requirements:

  • High school graduate or GED required. Bachelor's degree preferred in a human services field, foreign or domestic, or in a non-human services field with one year relevant job experience.
  • One year of direct service provision in human services/ navigation or a related field preferred
  • Certification: Medical interpretation certification or willing to obtain one.
  • Language fluency in English and Spanish.
  • Strong desire to serve refugee/immigrant communities.
  • Familiarity with needs of refugee/immigrant communities.
  • Ability to complete patient navigation training as needed.
  • Demonstrated cultural and linguistic competency.
  • Ability to serve as cultural broker and interpreter.
  • First hand cultural knowledge as a member/leader in local refugee community in Colorado.
  • Ability to build trusting relationships with community members and act as advocate for and liaison between refugee community and diverse network of providers.
  • Skilled at facilitating client and family engagement.


Required Vaccinations and TB Test.

AuMHC has prioritized the health and safety of our clients and staff and requires all employees to be fully vaccinated against Covid-19, receive an annual influenza (flu) shot, and receive negative TB test results prior to their first day of hire. Proof of both vaccinations and negative TB test will be required upon your first day of employment. Medical and religious exemptions may be requested for either COVID 19 vaccine or annual flu shot.
