Description
- Provides patient care, under the supervision of a Case Manager: Nurse or therapist.
- Provides personal care/companionship as directed by plan of care.
- Provides light housekeeping/ light meal prep tasks as directed by plan of care.
- Effectively communicates both verbally and in writing.
- Initiates and maintains positive relationships with patients/customers and co-workers.
- Observes and reports patient status changes including skin and psychosocial.
- Takes responsibility for self-development and supports a learning environment.
- Displays commitment to the mission of UHHS/UHHCS and its values.
- Successful completion of agency written test and competency evaluation of skills in the field within one month of hire and annually.
Qualifications
- High School Equivalent / GED.
- STNA or equivalent certification preferred.
- Valid drivers license required car insurance coverage.
- Minimum one year home care experience preferred.
- Current CPR certification or obtain within one month of hire.
- Completes required continuing education requirements to maintain certification.
