Job Summary

The Patient Care Coordinator RN or LPN helps to improve efficiency and communication among clinicians, physicians, and patients. They communicate with clinical and administrative staff to support the provision of safe and high-quality patient care. Primarily responsible for all telehealth communication and responsibilities.

Duties and Responsibilities

• Coordinate the implementation of services to ensure that quality service is provided
• Create, maintain, and update the clinician's schedules according to authorizations and patient needs
• Collaborates with staff and patients to identify open shifts, availability, and coordinate schedules
• Assist with Care Coordinators between the physician's office and Haven clinical staff
• Update, follow-up, manage, and monitor Infection Control Log

Qualifications, Knowledge, Skills, and Abilities

• Current Missouri RN or LPN license
• Ability to manage multiple tasks and prioritize assignments
• Familiar with regulations to meet Medicare/Medicaid conditions of participation and accreditation
• Ability to assist with completing patient admission documentation
• Proficient in computer skills and technology to complete documentation of clinical records
• Minimum of 1-year experience in home health care preferred
