Job Title: Registered Nurse (Hospice)

Shift: Day 5x8-Hour (08:00 - 17:00)

Job Description:

We are seeking a highly skilled and compassionate Registered Nurse with at least 1+ year of experience in home health/hospice nursing. The ideal candidate will have experience in geriatric/terminal patient care and possess an active license in the state of service - Maryland/DC. The primary responsibility of this role is to provide excellent patient care and support to individuals and their families in hospice care.

Responsibilities:

• Administer patient care and medication as prescribed by the physician.
• Assess the patient's condition and provide emotional support to the patient and their family members.
• Provide patient education and assist with end-of-life planning.
• Document patient care in an accurate and timely manner in the Electronic Medical Record (EMR) system.
• Collaborate with the interdisciplinary team, including social workers, chaplains, and other healthcare providers, to develop and implement a patient care plan.
• Attend to patient needs and requests in a timely manner.
• Maintain accurate patient records and communicate changes in the patient's condition to the physician and other healthcare providers.
• Comply with all healthcare regulations and policies.

Qualifications and Requirements:

• Active license in the state of service - Maryland/DC.
• Minimum 1+ year of experience as a home health/hospice nurse.
• Experience in geriatric/terminal patient care preferred.
• Must have EMR experience.
• Valid Driver's License, access to dependable vehicle, and clean driving record required.
• Mileage reimbursement between patients - .50 cents per mile.
• COVID Vaccination plus booster required - No exemptions accepted.
• All overtime must be pre-approved.
• Must be able to work the Day 5x8-Hour shift (08:00 - 17:00).
• NO on-call required.
