Case Manager
Intensive Case Management Services
Under a contract with the Los Angeles Department of Health Services, a West LA nonprofit community mental health agency has an immediate opening for organized, motivated, and professional candidates for FULL-TIME positions. The ICMS Case Manager will travel to multiple locations in the greater Los Angeles area to meet and work with members to attain and sustain permanent housing in partnership with the Department of Health Services (DHS). The Case Manager’s primary responsibilities are to provide Outreach, Engagement, Linkage, and Housing Location to adults who are homeless, coping with mental health, medical and substance use issues and in the process of approval for a subsidized housing voucher.
Essential Duties and Responsibilites include, but are not limited to the following:
- Intensive individual outreach & engagement of assigned homeless individuals
- Collect & compile all documentation needed to process housing application
- Housing location
- Ensure linkage to mental health assessment & treatment
- Ensure linkage to medical and substance use services
- Transport to/from appointments (when indicated)
- Document all delivered services
- Complete other duties assigned as needed
REQUIREMENTS:
- At least one year of experience working with homeless individuals AND a social work/mental health-related bachelor’s degree OR have a minimum of two years of experience providing direct mental health or intensive case management services.
- Experience working with clients with mental illness, chronic health issues, and substance use disorders.
- Knowledge and expertise in the following areas: chronic homelessness, outreach and engagement strategies, intensive case management services, best practice models, mental health and substance use disorder services, crisis intervention, suicide assessment and prevention, affordable housing and public benefits applications, housing and landlord/tenant rights, eviction prevention.
- Able to perform basic computer skills required for an electronic health record
- Evidence of clear criminal record and proof of valid California Driver’s License and Live Scan
- Bilingual in Spanish and English preferred
This is a Full-Time position with a 40-hour work week; the agency is currently open for services Monday through Friday 8:30 am -5:00 pm. The annual salary is DOE

Pay: $23.00 - $25.00 per hour
