This candidate must reside in New Hampshire. This is a remote position with the occasional need to travel to the New London office. The ideal candidate is self-motivated, tech-savvy and an excellent communicator.

We are looking for a team player who is willing to jump in and help, is flexible, able to prioritize and stay organized. A detail oriented Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our Intake Department for 36 hours each week.

Responsibilities for Intake Licensed Practical Nurse (LPN) or Registered Nurse (RN):

  • Accurately enter patient referral information into the electronic medical record system
  • Accurately enter all incoming patient care orders and notify appropriate team members as necessary
  • Navigate multiple electronic referral software portals
  • Communicate with discharge planners, physician offices and liaisons regarding patient referrals while maintaining positive relationships
  • Triage all incoming phone calls
  • Communicate effectively with patients and patient care givers

Requirements for Intake Licensed Practical Nurse (LPN) or Registered Nurse (RN):

  • Proof of up to date Covid-19 vaccination card to present at Orientation or engage in the exemption process prior to hire.
  • Per the CDC, "A person is up to date with their Covid-19 vaccination if they have received all recommended doses in the primary series and one booster when eligible."
  • Active NH (RN) Registered Nurse License
  • Active NH (LPN) Licensed Practical Nurse License
  • Must reside in New Hampshire
  • 2 years of Health Care experience
  • Triage Experience
  • Knowledge of Home Health/Hospice
  • Must be comfortable working in a remote environment
