Northern Human Services Community Services Center serving the Berlin and Colebrook areas is accepting applications for a full-time Transitional Service Coordinator position. Applicant must possess strong communication and organizational skills, be flexible, creative and enjoy working within a team.

A successful candidate demonstrates excellent written and communication skills, the ability to become familiar with many regulations and documentation requirements, and an understanding of the Agency’s vision and mission. The ability to work under pressure, handle multiple tasks simultaneously and attention to detail is essential.

High School diploma or equivalent is required. Prior relevant experience is preferred, but will train the right candidate.

Diverse responsibilities include assisting individuals with Developmental Disabilities and their families with the application and intake process, identifying appropriate adult services for incoming students and applicants, securing Medicaid, securing funding needs, establishing an appropriate transition plan in conjunction with the family and schools and acting as a resource to schools, Service Coordinators and community regarding transition to adult services.

This position requires a valid driver’s license, proof of adequate auto insurance and completion of driver’s, criminal and background checks, COVID-19 vaccination or approved, qualified exemption.
