• This is a PRN Home Health position covering the Cuyahoga area.
• Weekend availability is required.

The LPN Essential Duties Are:

• Assumes appropriate responsibility for patients assigned under the direction of an RN.
• Provides care according to the plan of care under supervision of an RN and within their scope of practice.
• Contributes observations and health information to the nursing assessment and reports all data to the RN.
• Provides care to meet the physical, emotional, spiritual and other needs of the patient/family according to the patient’s plan of care.
• Contributes to the evaluation of patient/family responses to the care provided to ensure patient/family comfort and safety.
• Educates patient and family on self-care techniques as directed.
• Administers medications per physician order using correct dose, route, and frequency to ensure accurate medication administration to patients.
• Documents activities carried out and observations made in the electronic health record and synchronizes this documentation for each patient in a timely fashion according to Pure Healthcare standards to ensure quality communication with patient’s Interdisciplinary Team.
• Recognizes and reports changes in mental or physical status of patient promptly to RN to provide sufficient communication to team members insuring patient safety and comfort.
• Assists in other activities and departments when requested to ensure complete and thorough patient care for all Pure Healthcare patients.
• Participates in Continuous Quality Improvement activities to support the quality of Pure Healthcare services.
• Precepts new staff to support professional relationships with newly hired team members as required.

LPN Qualifications:

• Registered LPN with the State of Ohio without any board actions
• One-year nursing experience preferred
• CPR Certified
• Computer skills sufficient to properly document services and care
• Ability to drive during daytime, nighttime or inclement weather
• Valid Driver's License with Safe Driving Record
• State Minimum Automobile Insurance Coverage
