Sun West Choice Healthcare & Rehabilitation located in Sun City West, Arizona is looking to fill a key leadership MDS Nurse position full-time. We are a skilled nursing facility with a clinical focus of behavioral, rehabilitation, and long-term care. We have the highest clinical staff-to-patient ratio in Arizona. We will continue to train and consistently hire to provide the best quality of care we can for our patients and maintain a properly staffed and well-cultured team.

LPN - Licensed Practical Nurse
RN - Registered Nurse

Licensure: Arizona LPN/RN License

Pay varies on experience and IS negotiable

We are located at 14002 W Meeker Blvd, Sun City West, AZ 85375

Duties:

• Assess and monitor patients' health in accordance with medical laws, regulations, and guidelines.
• Collect patient data and organize the information into reports for further research.
• Communicate with health care teams to create applicable health care plans for patients.
• Ensuring that patients have access to all the medical resources they need.
