Medical Social Worker

This position serves as an integral part of the after-hours team in the Social Work Department at a Hospital, serving patients in the Emergency Department and inpatient settings. This position is expected to work collaboratively with the Social Work Department, interdisciplinary medical teams at the hospital, outside community agencies, and diverse patient populations. The Social Worker assesses complex social situations including trauma, child abuse and neglect, family social needs, custody and guardianship issues, and responds to other psychosocial situations requiring social work services to assist with safe discharge of patient.

Department Specific Job Details:

• The position requires excellent assessment, communication, collaboration, intervention and documentation skills to address barriers and needs, identify risk factors, and provide connections to community resources and advocacy.
• Participates as a professional team member in patient care planning along with the hospital and community agency personnel.
• Provides education and consultation to health care professionals and the community regarding the impact of psychosocial factors.
• Is responsible for the delivery of social work services which are available to the patient, the patient’s family, and other persons significant to the patient.

Is well organized, directed, and integrated with other units and departments of the hospital and community. This position will collaborate with all divisions of the hospital.

The position is eligible for shift differentials for after-hours and weekend shifts worked. The schedule is four, 10 hour shifts, Friday through Monday. This position is eligible for full time benefits. Social Workers assist in departmental onboarding, training, coaching and job shadowing for newer staff. Participate in the Department’s peer mentoring program for new staff and provide mentoring around developing medical social work skills, and completed additional special projects as requested

REQUIRED:

Masters Degree in Social Work.

License through the State of Ohio Counselor, Social Worker and Marriage & Family Therapist Board, LSW or LISW.

PREFERRED: 1-2 years previous experience.
