Job description

Observation of clients, charting and reporting changes in clients' conditions, such as adverse reactions to medication or recording a change in treatment and taking any necessary action.

Record withdrawal symptoms, and record collected data.

Collect UA samples and prepare for the lab

Provide collected medical data to clinical staff.

Document medication observations per company policy

Educate clients regarding prescriptions and medication management. (Licensed Personnel Only)

Knowledge of detox medications, and different types of substance abuse withdrawal symptoms.

Ensure accurate documentation on medication observations orders and monitor medication audits/counts and inventory.

Record data and communicate with physician for medical clearance in relation to treatment, planning and discharge.

Measure and record client's vital signs.

Observe client concerns and recommend client to emergency medical care when necessary. Perform inventory check and maintenance of nursing supplies and over the counter medication supplies, responsible for requesting inventory and requisition supplies/instruments.

Client documentation and summary reporting at shift end.

Client rounds 1 time per hour, dependent upon census/staffing.

Maintain compliance with HIPAA regulations, ADA and Joint Commission policies and procedures boundaries and client and company confidentiality.

Fully understand and maintain policies regarding professional ethics, including appropriate attire.

Other duties as requested by leadership.

Attends in-service training, educational workshops, and staff meetings.

Assist client with filling/refilling prescriptions.

Assist client with making outside appointments for medical care/management.

Coordinate transportation for outside appointments/ordered labs.

Perform PPD testing as ordered by physician (Licensed Personnel Only).

Coordinate payment to pharmacy for medications with clients.

Document outside appointment information into client chart and follow up with prescribed regimen.

Witness medication destruction with pharmacy representative.

  • Must have valid California driver’s license and liability insurance if driving personal vehicle on Center business.
  • Vision, hearing, manual dexterity and hand-eye coordination must be adequate for performance of job duties. Able to sit at desk, use keyboard, write and physically perform other job duties. Able to move about the facility to observe clients and staff.
