We are currently looking for a full-time Nurse Care Manager.

General Description

In addition to suffering from mental health issues, a lot of our clients have co-occurring chronic health conditions as well. The Nurse Care Manager will Collaborate with the treatment team members to provide integrated health and nursing services to persons served of the assigned teams.

Job responsibilities:

  • Collaborate with agency nurses to ensure that clients receive injectable medications in timely manner.
  • Collaborate with team on the monitoring of blood pressure, glucose monitoring, and tests related to monitoring chronic diseases, and education of staff to provide this service to clients.
  • Oversee, with the team supervisor, team medication management process, per agency policy.
  • Educate team members and clients on medications and side effects, nutrition, management of chronic physical health diagnoses, as needed and per the client’s treatment plan.
  • Provide basic first aid and assessment/intervention in medical emergency situations
  • Provide consultation to staff and clients to ensure that measureable, relevant, and realistic physical health goals are in treatment plans and routinely revised (at least quarterly) to reflect changes
  • Conduct annual physical health assessments with clients, including physical health recommendations
  • Collaborate with client and team to create measurable physical health goal for treatment plan
  • Communicate health indicator data received from healthcare home monthly, that are outside of goal, to team and clients, and revise treatment to reflect action plan; document any barriers to treatment in a progress note
  • Coordinate enrollment of team clients in HCH
  • Collaborate and track annual completion of metabolic screenings for team clients; Education of results of metabolic screening is to be provided to each client and recommendations based on results included in the clients treatment plan or treatment plan revision.
  • Provide medication reconciliations with a client within 72 hours of discharge from an inpatient admission
  • Collaborate with team members and other professionals in the organization
  • Collaborate with Physical Health Integration Team for support and to enroll one client a month in the Primary Behavioral Health Care Project
  • Participate in team meetings, treatment planning meetings, and medical services meetings
  • Advocate with external physical health providers for clients to receive standard of care and to communicate the client’s health goals to their primary care provider
  • Maintain and improve professional skills by attendance at in-service and appropriate training seminars
  • Completes annual training requirements as outlined in the Employee Handbook
  • Performs other related tasks/jobs as assigned by the supervisor

Job Qualifications and Requirements

Requirements: Registered Nurse. Six months of psychiatric nursing experience. CPR certification, Personal vehicle, Chauffeur’s License, and valid auto insurance.

Physical Requirements: Ability to stand, bend, kneel, lift (20-30 lbs), navigate steps or use appropriate accommodations, drive and be able to sit for periods for hours at a time.
