Nurse Care Manager / Registered Nurse – Behavioral Health Community Partners
Riverside is seeking a Registered Nurse to join our growing Behavioral Health Community Partners (BHCP) team in Dedham. We have a unique opportunity to expand your interdisciplinary team and coordination with clients living with psychiatric or mental diagnosis.
The BHCP team will be providing services to people served in the community. In this exciting position, you will be part of a multi-disciplinary team, collaborating with behavioral and medical health providers all working to link eligible Masshealth consumers to community based resources and to reduce barriers to these consumers engaging in appropriate community based services.
The Nurse Care Manager will provide direct care management, oversight of medication reconciliation and coordination to enrollees, collaborate with medical and other care providers, provide consultation to BHCP team members and consumers regarding appropriate behavioral and medical health services of the team’s enrollees, and participate in the ongoing development of trainings for staff and enrollees regarding health and health behaviors.
Excellent flexible schedule: Monday through Friday 9:00 a.m. - 5:00 p.m. Nights, weekends, holidays and on call not required.
This position is currently remote with the intention to move back into the community when we are able to do so.
Salary: $75,000/year

Required Skills
- Registered Nurse with current MA license to practice
Required Experience
- Experience with people with mental illness required; experience with substance use disorders preferred
- At least one year of providing care to individuals with medical and/or behavioral health diagnoses strongly preferred
- Valid drivers’ license and regular access to reliable transportation required
