Responsible for managing patient care and treatment in collaboration with the physician.

Primary duties may include, but are not limited to:

• Performs health assessments and preventative health measures within prescribed guidelines and physician instructions.
• Orders, interprets and evaluates diagnostic tests to identify and assess patient's clinical problems and health care needs.
• Discusses case with physician.
• Formulates and documents care plan.
• Prescribes medication or other forms of treatment.
• Participates in On-Call program which may include after-hours, weekend and holiday calls and visitations.
• Rounding at assisted living facility and board and cares but charting from home.
• Outreach for information via phone and email

Requirements:

• Requires an MS in Nursing; 1 year of related experience with clinical emphasis for nurse practitioner; or any combination of education and experience, which would provide an equivalent background.
• Current unrestricted RN license and NP license in applicable state required.
• Credentialing,
• Primary Source Verification
• Cactus, Excel, NCQA, CaQH preferred.

Additional Details:

• Self scheduling and visits within a part of Santa Clara Market.
• Current team has 80-90 patients panels and 12 buildings. (No home visits)
• Position is field based and will be rounding on patient is the Assisted Living, Board and Cares, and Nursing Facilities with in Santa Clara County
• Travel time, 75% (reimbursement provided)
• Monday- Friday 8am-5pm with occasional week night and weekend on all schedule (nights and weekends would be remote)
