Summary- The Patient Coordinator/Marketing Team Member will be responsible for transferring new participants to Holland Home Care, LLC services as well as visiting/planning marketing visits to local communities in order to increase our participant census.
Patient Coordinator
- u Must have efficient experience with HHAexchange
- u Conducting enrollment calls to Maximus/IEB when necessary
- u Creating and maintaining participant’s master calendar on HHAexchange
- u Following up on participant’s authorizations
- u Assist in confirming electronic verification visits and/or time sheets through HHAexchange
- u Eligibility verification through all appropriate governing agencies
- u Successfully completing monthly quarterly in-home participant visits
- u Auditing and maintenance of all pending new intake files
- u Assist in completing all signage (including participant service agreement prior to the start of service) when appropriate.
Marketing Team Member
- u Assisting with scheduled marketing events, including “HHC Street Team” in order to increase our visibility in the community which will create opportunities to grow our participants as well as our network of qualified caregivers.
