Patient Care Coordinator Position Summary

The Patient Care Coordinator demonstrates knowledge and skills in the care of our patients. The Patient Care Coordinators work as a team to facilitate care for our patients and take daily directions from the doctors and the physician assistant in the coordination of clinical and administrative duties related to the care of our patients. The Patient Care Coordinators are also representatives of our group in communicating and collaborating with other physicians' offices, Hospitals, Surgery Centers, Pharmacies, Laboratories and other ancillary facilities. Position can be based at either the Redwood City office or Mountain View office or both.

Essential Functions of this Position

  • Register and schedule new patients by phone or online.
  • Assist patients to set up online accounts and fill out information prior to meeting with the doctor
  • Track and audit all patient consent forms
  • Assist and facilitate telemedicine sessions
  • Schedule follow up visits, procedures ordered by the doctors, provide information regarding lab or x-ray tests and prescriptions
  • Transcribe, enter and verify data from source documents into the electronic health record (EHR) in an accurate and timely manner.
  • Route and file documents in EHR accurately and promptly
  • Serve as a liaison between patients and doctors, relate messages, instructions and some test results via secure portal.
  • Communicate with pharmacies regarding new prescriptions and prescription refills as directed by the doctors and the PA.
  • Answer patient calls and emails that are within scope of practice and document action taken in patient EHR.
  • Check and reply to voicemails
  • Work with the Practice Manager to act as a liaison for difficult situations and patient issues.
  • Provide information and patient education regarding procedural preparations, directions, and all logistics
  • Collaborate with team members at our surgery center to ensure all GI procedures are scheduled properly

Required Qualifications for this Position

The right candidate will have strong communication skills, the ability to multi-task and operate in a dynamic environment, operate both independently and as a part of a team environment, and bring a positive attitude to the workplace environment.

  • Bachelor's degree, Associates degree or other related technical education preferred.
  • Previous work in a medical office preferred.
  • Strong computer skills, ability to use basic office equipment.
  • Proficient in Electronic Medical Records, esp. EPIC is highly preferred
  • Strong ability to follow procedures is essential.
  • Must be organized, with a strong ability to multi-task, prioritize, and have a strong attention to detail and strong ability to utilize time efficiently.
  • Must have a strong ability to act independently while under the supervision of the practice manager.
  • Must have excellent customer service skills, exhibiting courteous, compassionate and respectful treatment of internal and external customers at all times.
  • Must consistently display a positive attitude and flexibility in changing situations.
  • Must have excellent communication skills and make it a priority to consistently communicate positively and professionally with all staff members and patients.
  • Demonstrate the ability to be a team player and possess a willingness to perform duties that are assigned or delegated within a timely manner.
  • Participate in identifying problems and suggesting solutions.
  • Maintain confidentiality of all work information.
  • Maintain strict confidentiality for all patient and clinic information, consistent with established security and confidentiality policies and HIPAA regulations, at all times.

Compensation Range: $20 - $25+ per hour to start, commensurate to level of experience
