Patient Care Coordinator
Job description
- Obtaining information to verify insurance coverage and inform the patients
- Identifying primary and/or secondary payer of services
- Participating in establishing the plan of treatment through direct
- Explain the function of the home health team members who will implement the plan of treatment to the patient
- Developing and coordinating health care programs
- Communicating between staff, patients and family
- Scheduling shifts
- Handling patient case management and education
- Recruiting and training staff and creating schedules
- Developing patient goals and monitoring progress
- Maintaining patient confidentiality and quality care
Job Types: Full-time, Part-time, PRN
Pay: $18.00 - $20.00 per hour
