Job Description

Purpose of Position:

The function of the Patient Representative under supervision of the Asst. Director of Patient Relations, interprets hospital philosophy, policies, procedures and services to patients, families, significant others and visitors.

Job Responsibilities:

- Ensure that patients, families, significant others, visitors and staff are aware of patients' rights and how they may be exercised.

- Convey Patient Advocacy's role to patients, families, significant others, staff and visitors.

- Education patients on New York State Advance Directives.

- Listen to patients' problems and concerns.

- Evaluate patients' problems and concerns to determine appropriate action to resolve them.

- Document all complaints, requests and compliments concerning the institution, staff and services in VOICE.

- Refer to relevant areas for investigation, obtain findings, and communicate resolution to patients, families, significant others and visitors, in writing within established deadlines.

- Keep patients fully informed of the status of issues.

- Consult with appropriate staff as part of the resolution process to ensure corrective measures are taken to resolve the issues in a timely and effective manner.

- Assists in the completion of Health Care Proxies.

- Performs all other duties assigned by Supervisor and/or designee.

Minimum Qualifications

1. Master’s degree from an accredited college or university in Public Health, Public Administration, Hospital Administration, Business Administration or in a health care specialization or related discipline; or
2. Bachelor’s degree from an accredited college or university in fields, as described in “1” above or in social sciences; and one (1) year of paid, full-time responsible experience in an administrative, supervisory or consultative capacity performing functions related to the coordination and delivery of patient care services or similar functions in a hospital or a health care setting; or
3. A satisfactory equivalent of education, training and/or experience.
