SCOPE OF POSITION
The Pharmacist, Clinical Specialty Services shall be responsible for the provision of specialty and clinical pharmacy services/pharmaceutical care as delineated by the service’s Scope of Care for the patients of the entity and shall practice under the direction of pharmacy leadership and the guidance of professional pharmacy standards. The candidate will assure the appropriate selection, dosing, administering, and dispensing of medication orders and serve as a subject matter expert in the pharmacy discipline and provide information to patients, providers, and leadership. The candidate will oversee technical personnel who assist in the preparation and dispensing of medication and those who may assist with financial assistance and authorizations within the department. The candidate will serve as the pharmacy liaison between the specialty pharmacy, specialty clinics, and patients. The individual will be responsible for maintaining competency in the unique aspects of this practice. This role will be responsible for clinical practice, patient monitoring, and documentation based on the practice needs, accrediting agency, and regulatory guidelines for specialty pharmacy. The candidate must complete all patient documentation and care in a timely manner to achieve best possible clinical outcomes by supporting patients in their therapeutic journey. The individual will maintain patient list, coordinate refills/deliveries, and schedule appropriate follow-ups to access medication safety and efficacy and provide reports to provider. This pharmacist will act as financial advocate for patients when needed, leverage charitable foundations, state programs, manufacturer programs to ensure medication affordability, and/or facilitate prior authorizations, appeals, denials. The candidate will maintain all patient and auditable records. The Pharmacist, Clinical Specialty Services will assist with academic functions such as serving as a preceptor for pharmacy and/or medical students or residents. The individual will participate in departmental process improvement initiatives, participate in the expansion of pharmacy services, and perform other duties as assigned.

EDUCATION AND EXPERIENCE
The candidate must be a graduate of a college of pharmacy accredited by the American Council for Pharmaceutical Education.
The candidate shall have at least five (5) years of clinical pharmacy experience and/or a minimum of 2 years of specialty area experience OR have successfully completed a post graduate residency with a dedicated specialty area component. An advanced pharmacy degree with an applicable residency OR comparable experience (as delineated previously) accompanied by Board Pharmaceutical Specialties Certification in at least one discipline (e.g, BCPS, BCOP) shall be required (BCPS required within 36 months of hire).
Previous experience/proficiency in the provision of education and training of pharmacy or medical students is preferred.
This individual shall possess strong communication and analytical skills as well as leadership abilities. The candidate must demonstrate the ability to work closely with physicians and paramedical professionals in a multidisciplinary setting.
Pharmacist shall possess abilities and knowledge to function on the consult service, collaborating professionally and effectively with team members (including physicians) regarding patient care needs and with a goal of positive, patient-oriented outcomes.
Must hold current licensure by the appropriate state Board of Pharmacy. (TN, VA)
BLS designation must be attained and maintained.
License/Certifications
A graduate of a college of pharmacy accredited by the American Council for Pharmaceutical Education.
