MaximHealthcare Services is looking for a Registered Nurse or Licensed PracticalNurse to provide direct care for people in their homes.

Responsibilities:

• Utilizes the nursingprocess to assess, plan, implement and evaluate patient care
• Assess signs andsymptoms indicating physiologic and psycho-social changes in the patient’scondition
• Collects, analyzes,and interprets data and information from health care members
• Documents thepatient’s care plan using identified nursing diagnoses, expected patientoutcomes, and selected nursing interventions
• Performs interventionsaccording to identified priorities, care plan, and the hospital policies andpatient care outcome standard
• Revises the plan ofcare according to evaluation, changes in medical care plan, andeffective/ineffective nursing interventions
• Uses clinical judgmentin evaluation activities to meet patient care

Minimum Requirements:

• Current nursing license for the state in which the nurse practices or compact state
• One (1) year nursing experience in a long term acute care facility preferred
• Ability to effectively elicit/provide information to and from appropriate individuals(including, but not limited to, supervisors, co-workers, clients) via strong communication skills proficiency in the English language may be required
